MEDICATION PROCEDURE:
THE PROCEDURE IS CLEARLY OUTLINED ON THE DISTRICT'S MEDICATION FORM.
No child is to bring medication of any kind
to school unless accompanied by a note signed by the doctor AND parent. The
medication must be delivered to the school in a LABELED PRESCRIPTION bottle. The
label shall contain the child's name, the name of the drug, dosage and the
physician's name. Pills in bags or unmarked bottles will NOT be given. If
an over-the-counter drug is to be given, a doctor's order AND parent's
permission are still required. The medication must have the child's name written
on it and be in its original container. Physician & parent authorization for
medication administration will remain in effect for one school year only. ANY
change in dosage requires completion of a new Physician's order and parent
request.
For safety reasons certain medications such as,
but not limited to Ritalin, Clonidine, Adderall, etc. may not be carried by the
student & must be delivered to the school by an adult. If a student must
have their medication in their possession, it must be so stated in the
Physician's order. We are here to meet your child's health needs. We will work
with you, and your child's physician, to determine what accommodations need to
